Format. Usually this will be DVD
Distributor: this is usually a company name, Capitalise each word because it is a title i.e. Madman.
Place: this is the location where the production/distribution company is registered.
Special credits of other information: if you think your teacher will need more information to tell exactly which version you used include it here.
Name of organisation year of posting, title of webpage, Sponsoring body, place, Date accessed in day month year format, <URL inside greater and lesser symbols>.
